The Confidentiality Agreement (the "AGREEMENT") is made as of this first day a patient agreed to accept treament in our hospital ("Effective Date") by and between Amen herbal Hospital (the "HOSPITAL") and the Patient.

(The Hospital and the patient are hereinafter referred to individually as a "Party" and collectively as the "Parties".)

WHEREAS, the Hospital desires to treat the patient and the patient desires to accept the treatments such services upon the terms and conditions set forth below.

NOW, THEREFORE, in consideration of the premises and of the mutual covenants and
agreements hereafter set forth, the Parties agree as follows:

1.All medical reports and records will be held confidently. 

2.All medical reports and records will be given to the other party("The Patient") upon request.Therefore,  Any medical report leakage, the hospital will not be held responsible.
